Looking for registration forms? We're here to assist you! Simply complete the new patient packet we sent via email after you booked your appointment. If you prefer paper, just give us a call at 763-383-1788, and we'll mail them to you. Please arrive 10 minutes early for your appointment with fully completed new patient forms. Failure to follow these instructions will result in our team rescheduling your visit to a later date. Late arrivals will not be accommodated.
